And innovative grant from the Foundation has grown into the Patient Navigator Program in the Emergency Department at Saint Francis. The Patient Navigator links patients to valuable community resources like shelter, primary care, treatment and recovery services and more, to keep people healthy so they are less likely to require emergency treatment.
